NHNP physicians have identified prevention as a key factor in improving overall patient health. Prevention is defined as the early detection and treatment of disease. Our ACO doctors have specially designed disease programs (i.e. CHF, COPD, and diabetes) that are greatly improving patient quality of life.

PATIENT ENGAGEMENT

Patient engagement and engaging the patients’ families are key factors in improving patient health outcomes. Patient Engagement in an ACO has two primary goals: expand the doctor-patient relationship and educate the patient. ACO doctors attempt to share the decision making process with their patients and well-informed patients are more likely to follow through with their joint treatment decision. In addition, active involvement with patients and families can lead to lower underuse or overuse of medical services, reduced patient health care costs, and improved health outcomes.

CARE COORDINATION
